As our Geriatrics Social Services Coordinator, you’ll have a key role in enhancing the well-being and quality of life for our residents with compassionate care coordination and community-building.
This position requires a bachelor’s degree in social work with direct and recent geriatrics experience OR five (5) years of experience as a Certified Geriatric Case Manager.
In this vital position, you will coordinate assessments, planning, and access to services, ensuring residents receive the support they need to thrive. Working closely with cross-functional teams, you'll help residents meet Conditions of Occupancy while fostering a strong sense of community.
